Admissions

To make a referral or to request an evaluation, please contact our Admissions Department at info.resolute@yfcs.com or call 317.630.5215.  Inquiry calls made after business hours will be followed up the next business day. 

Once the child has been accepted, the Admissions Coordinator will contact the placing agency with a notice of acceptance and will request the following documents for completion and/or review at the time of admission:

Ø      Placement Agreement

Ø      Placement Packet

Ø      Court Order placing the child at Resolute

Ø      Copy of Social Security Card

Ø      Copy of Birth Certificate

Ø      DCS Case Plan

Ø      Insurance and/or Medicaid Card

Ø      Individualized Education Plan

Ø      Immunization Records

Ø      Interstate Compact (for out of state placements only)

If the child has not already been assessed by Resolute, we will also request additional documentation to determine if the child is appropriate for placement including: probable-cause affidavits, predisposition reports, 310/311 reports, and/or previous treatment history/records.

At the time of admission, belongings should be brought with the child to the facility.  The Admissions Coordinator will assist each family in understanding what items are needed during the treatment stay.  Transportation can be provided by Resolute upon request.  It is encouraged that families take part in the admissions process whenever possible and, as such, the Admissions Coordinator will make arrangements with the family to schedule a facility tour and make staff introductions unless otherwise requested.  All documentation should be provided prior to the child’s arrival at the facility.
